To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please be prepared to pay your patient portion or deductible at the time of your treatment visit. Our staff will help determine these at the time you complete your visit. As a courtesy, our office will file your insurance form reflecting the treatment information to your insurance company electronically.
If you choose to pay your treatment fee in full, our office will arrange to have the insurance benefit mailed directly to you.
To help us to be able to communicate with your insurance company, please bring your insurance information ( identification card or completely filled out insurance form) with you to the initial visit so that we can expedite reimbursement.
At your future visits, don’t forget to let us know if you have any changes in insurance coverage, companies, employer, deductibles and contact information. We will be happy to update our records!
