First Visit

Your Initial Appointment

first-visit

During your first visit, Drs. Mogelof welcomes you to see our state-of-the-art office and meet the staff of The Mogelof Dental Group, LLC . After completion of several short forms, you will be introduced to Dr. Andrew or Dr. Scott Mogelof. If you are beginning with a hygiene visit, you will have a thorough cleaning, examination by the hygienist and then by the doctor. We use an intra-oral camera and digital x-rays to make sure we communicate the findings and answer any questions you may have.

If more complex issues are found, the doctors will schedule a separate consultation visit so they are able to sit with you without interruption to discuss your needs.

We make every effort to treat our patients on a “one on one” basis in an unhurried manner.

In cases of complex medical history or treatment plan we will require an evaluation and a second appointment to provide treatment on another day.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the initial visit.

Please assist us by providing the following information at the time of your initial visit:

  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

Please alert the office if you have a medical condition that may be of concern prior to your treatment or surgical procedure (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your prior dentist  has taken x-rays within the last 12-24 months, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office at the time of your appointment. If additional films are necessary, they can be taken at our office because we always want to rely on the most current and properly diagnostic x-rays.
